Nelle will probably try to get under Sam's skin at some point, but her being Sam's stepmother has no real ramifications.

Sam has a very limited relationship with Julian for good reasons. When she finds out that Julian has married Nelle, she will only be more disgusted with him and have greater incentive to avoid him.

And if Sam knew the truth, that Julian had nearly killed Lucas and that he knew Michael was Wiley's father, she'd hit the roof.

As for Nelle being step-grandma to Danny and Scout, that's also just on paper. Julian has no relationship with his grandkids and Sam certainly isn't going to allow Nelle access to them, even if Nelle wanted to see them (which she has no reason to).
